| 2012 - 2013 Financial Aid Dual Enrollment Authorization Form, Kekaulike Information and Service Center (PDF Form)
Applicants taking at least 6 credits at Kapi'olani Community College, KCC is your home institution, and will be concurrently enrolled at another UH system school may be eligible for financial assistance. You must meet with an academic advisor to complete this form to confirm that the credits will transfer back to KCC towards your current degree you are seeking.
